A Special is an unlicensed medicinal product manufactured or imported to meet the special clinical needs of an individual patient. Where the licensed market cannot provide a suitable product, Specials fill the gap. They are governed in the UK by Regulation 167 of the Human Medicines Regulations 2012.

Most Specials fall into three categories. Bespoke formulations, such as a liquid version of a tablet for a child who cannot swallow. Strengths or presentations that are not commercially available, such as a 12.5mg tablet of a drug only sold as 25mg. And imported products, where the medicine holds a licence in another country but not in the UK.

A licensed product, where one exists and is clinically suitable, must always be the first choice. Specials are reserved for cases where the licensed catalogue cannot meet the patient's clinical need. Euro Biom supplies only against a prescriber request that documents this clinical justification.
